Event Participation Release & Waiver of Liability

All players 18 years old and above must read and agree to this term, minors must be registered by parent/guardian.

Please read this form carefully. By checking the box in registration you will be waiving and releasing all claims arising out of your participation in this event. In consideration of Cada Casa & Roots Futsal providing this event and accepting me as a participant in this event, you (or parent/guardian) fully agree, without exception, to the following:

I am fully informed of the details of this event and have received satisfactory answers to all questions I have concerning this event. I do hereby assume the full risk of any injuries and all costs, damages, and losses that I may sustain as a result of participating in any and all activities associated with this event.

I grant to Cada Casa and Roots Futsal the right to record, broadcast, and otherwise use in any media my participation in this event and to use my name, likeness, voice, and biographical information.

I assume all risks associated with my participation in this event and hereby release and hold harmless Cada Casa & Roots Futsal, the sponsors and suppliers of this event, and their respective directors, officers, employees, agents, successors, and assigns, from and against any and all claims, damages, liabilities, and expenses arising from my participation in this event. I have read and fully understand the foregoing terms.
